About The Position

The Care Coach evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Coach 1 work assignments are often straightforward and of moderate complexity. The Care Coach will work directly with members enrolled in the Humana Beginnings program and may require in person visits with members. Care Coaches will employ a variety of strategies and techniques to support a member's optimal wellness state by coordinating services & resources. Identify and resolve barriers that hinder effective care. Ensure patient is progressing towards desired outcomes by monitoring patient care through use of assessment, data, conversations with member, and active care planning for prenatal and postnatal members. The Care Coach role is a Field position where travel is required. Travel will be up to 25% of the time within the state.

Requirements

  • Must be a South Carolina Resident
  • LPN, RN or bachelor's degree in health and human services field
  • 1 or more years of maternity experience
  • Knowledge of Microsoft Office Word and Excel
  • Exceptional verbal/written communication and interpersonal skills
  • Must have a separate room with a locked door that can be used as a home office to ensure continuous privacy while you work
  • Valid state driver's license
  • Carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for base liability coverage or whichever is higher.

  • Prior experience with Medicare & Medicaid recipients
  • Previous Care management experience
  • Previous experience with electronic case note documentation and with documenting in multiple computer applications/systems
  • Experience with health promotion, coaching and wellness
  • Knowledge of community health and social service agencies and additional community resources
