About The Position

In response to the growing and increasingly diverse health and wellness needs across Weld County, the Weld County Department of Public Health and Environment launched the Public Health Mobile Unit in 2025. This innovative initiative was designed to reduce barriers to care for residents in rural communities and underserved neighborhoods who often face challenges accessing traditional health care services. Developed through thoughtful planning, collaboration, and community engagement, the Mobile Unit functions as a mobile extension of Weld County’s public health clinics, bringing essential services directly to the people who need it the most. It reflects a shared commitment to equitable, preventative, and community-centered care, ensuring residents are met where they are, both geographically and holistically. As part of this effort, North Range Behavioral Health partners with Weld County to integrate mental and behavioral health services into the Mobile Unit’s offerings. This partnership recognizes that mental health is a vital component of overall wellness and that accessible, timely support can significantly improve individual and community outcomes. Together, North Range Behavioral Health and Weld County are expanding access to care, strengthening community connections, and improving health outcomes across the county. The Bachelor-level Mental Health Counselor is central to this effort, bridging public health and behavioral health, reducing gaps in care, and reinforcing a shared vision of a healthier more resilient Weld County. This position includes eligibility for a monthly performance bonus.

Requirements

  • Bachelor’s Degree from an accredited college or university in psychology, social work, counseling, or another human services degree.
  • At least one year of experience working with individuals with mental/behavioral health challenges.
  • Bilingual English/Spanish speaking/writing
  • Can successfully complete pre-employment screening, including background check, fingerprinting, and drug and TB testing.

Responsibilities

  • Conducting initial and ongoing assessments to identify mental health needs and strengths
  • Provide crisis intervention, short-term counseling, and individual, family, and group therapy
  • Deliver case management services to support client stability and continuity of care
  • Develop strength-based service plans, monitor treatment progress, and adjusting interventions as needed
  • Maintaining accurate electronic clinical records, documentation, and required reporting
  • Coordinate care with internal teams and external community partners to address social, medical and behavioral health needs.
  • Collaborate closely with clinical supervisor, multidisciplinary team members, and a psychiatrist to ensure high-quality, coordinated treatment.
  • Through this role, North Range Behavioral Health helps ensure that residents accessing the Public Health Mobile Unit receive compassionate, culturally responsive mental health care alongside services such as vaccinations, STI testing, maternal and child health support, health education, and screenings.
