About The Position

Theoria Medical is a leading healthcare innovator serving the post-acute sector, specializing in multispecialty physician services for skilled nursing facilities nationwide. We are seeking a Medical Director and Attending Physician to serve local skilled nursing facilities within a mission-driven care community. This role allows physicians to care for patients over time, building trust and making a lasting difference for vulnerable populations.

Requirements

Physicians (MD/DO) must hold an active U.S. medical license.
Must be board-certified or board-eligible in an appropriate specialty (Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine are examples).
New graduates who are board-eligible and actively pursuing licensing are encouraged to apply.
Must be able to perform the essential functions of this position satisfactorily, with or without a reasonable accommodation.

Responsibilities

Serve as the attending physician for patients in each assigned SNF.
Collaborate with NPs/PAs to manage daily patient care.
Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
Participate in Theoria’s Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
