Requirements

  • A high school diploma/GED AND one year of experience working with people with developmental disabilities.
  • Certified Nursing Assistant (NAC) Certification is required to work in the facility.
  • Successfully complete employer providing NAC training.
  • Must be able to pass state exam and clinical competency test
  • Must have or be able to obtain and maintain a Nursing Assistant Certification license (NAC)

Responsibilities

  • Provide care to male and female residents under the clinical direction of an RN or LPN and supervision of an Attendant Counselor Manager, including bathing, dressing, grooming, feeding, toileting, personal hygiene, measuring weight, and recording food and fluid intake/output
  • Assist in meal preparations adhering to diet orders, differentiating food and liquid textures/consistencies, and may need to assist in feeding a resident, recording intake and output
  • Perform resident-specific transfers, repositioning, and transportation, utilizing facility equipment appropriately, and follow standard precautions to prevent disease transmission
  • Serve as relief shift charge, assigning staff duties, ensuring training, and assisting clients with Activities of Daily Living (ADLs) and personal care
  • Monitor and report clients' medical and overall conditions; function as a member of the Interdisciplinary Team (IDT) to develop, document, and implement formal programs
  • Transport clients to appointments and activities; provide money management opportunities when appropriate; ensure client safety, security, and protection
  • Transfer, reposition, and transport clients within the cottage and campus; perform light housekeeping duties and launder personal clothing and bedding; manage inventory of supplies and personal possessions
  • Implement individualized programs and services designed to maintain skills and facilitate independence, in accordance with the Individual Plan of Care (IPOC), collecting and recording data as needed
  • Observe, document, and report changes in clients' physical or mental conditions and unusual behaviors; report and ensure completion of incident reports for abuse, neglect, or injuries of unknown origin
