About The Position

The Attachment Outreach Worker supports community outreach initiatives designed to improve awareness of and access to primary care services across Hastings and Prince Edward Counties. Working in partnership with healthcare providers, community organizations, municipalities, and residents, the Attachment Outreach Worker will engage directly with community members to provide information about primary care services and attachment opportunities. This position is ideal for a student interested in healthcare, community development, public health, health administration, nursing, social services, or related fields.

Requirements

  • Strong interpersonal and communication skills.
  • Comfortable engaging with diverse populations and speaking with members of the public.
  • Knowledge of or interest in the Ontario healthcare system.
  • Ability to work independently and as part of a team.
  • Strong organizational and time-management skills.
  • Valid driver's licence and access to reliable transportation.
  • Willingness and ability to travel throughout Hastings and Prince Edward County.

Responsibilities

  • Conduct outreach and engagement activities in a variety of community settings, including health fairs, farmers' markets, festivals, grocery stores, shopping malls, libraries, community centres, post-secondary institutions, seniors' residences, faith-based organizations, municipal facilities, and other high-traffic public locations throughout the Hastings Prince Edward region to increase awareness of primary care access.
  • Educate residents about how to access primary care and register with Health Care Connect.
  • Support outreach efforts targeting unattached patients and underserved populations.
  • Distribute educational materials and resource information.
  • Represent the organization professionally in community settings.
  • Support collaborative initiatives with healthcare and community partners.
  • Track outreach activities and maintain records of events attended and individuals reached.
