About The Position

The Assistant Director of Health and Wellness is responsible for overseeing the health and wellness of residents, supervising health services personnel, and ensuring compliance with all relevant regulations and company policies. This role involves conducting health evaluations, monitoring resident health status, coordinating care conferences, investigating incidents, and fostering a positive and safe environment for residents and staff.

Requirements

  • Must have a thorough health evaluation on all new residents upon move-in.
  • Must monitor the health status of all current residents through evaluations and care conferences scheduled quarterly or more frequently as required.
  • Must monitor resident evaluations and service plans for team compliance.
  • Must react immediately to suspected resident abuse including a thorough investigation and notification of appropriate parties.
  • Must supervise all Assisted Living and Memory Support personnel.
  • Must provide team member evaluations per Company policies and procedures.
  • Must communicate all employee relations issues to Executive Director.
  • Must perform all necessary verbal and written disciplinary actions required of supervised staff.
  • Must attend daily management meetings, staff meetings, and other meetings as delegated by the Executive Director.
  • Must work with Executive Director and The Support Center to conduct community audits as required.
  • Must review all incident reports, conduct investigations and reports/plans corrective action as required.
  • Must work with Executive Director to ensure there is Continuous Quality Improvement (CQI) activity in the building.
  • Must work with Executive Director to identify and set priorities to resolve service-related issues.
  • Must support Executive Director in maintaining compliance with all local, state, federal, and licensing rules and regulations.
  • Must ensure all employees work in a safe manner and that unsafe actions are managed.
  • Must foster teamwork with co-workers, show initiative in overcoming obstacles, take actions which assist others beyond job responsibilities, and have a positive impact on co-workers, residents, and visitors.
  • Must perform other duties as assigned or needed.

Responsibilities

  • Performs thorough health evaluation on all new residents upon move-in.
  • Monitors the health status of all current residents through evaluations and care conferences.
  • Monitors resident evaluations and service plans for team compliance, including medication side effects, programming, meals, complaints, cognition and behavioral expressions, ADLs, and special requests.
  • Reads all alert charting from daily reports and initiates actions to improve resident health status.
  • Coordinates and attends resident care conferences and service plan reviews.
  • Reacts immediately to suspected resident abuse, including investigation and notification of appropriate parties.
  • Supervises all Assisted Living and Memory Support personnel, including hiring, coaching, orientation, scheduling, and training.
  • Provides team member evaluations per Company policies and procedures.
  • Communicates all employee relations issues to the Executive Director.
