About The Position
The Associate Discharge Planner is a remote, per diem position working 8-hour day shifts. Under the general supervision of the Director, this role supports the care management team by coordinating discharge planning and assisting with departmental operational functions. The goal is to ensure efficient transitions for patients from the hospital to home or alternative care settings. The position is with Providence California Regional Services, whose culture emphasizes patient-focused, whole-person care, understanding, commitment, mutual respect, and empowering caregivers.
Requirements
- Clinical or administrative experience in the healthcare industry.
