About The Position

Supports patients with dementia and their caregivers. Experience with ageing population care and caregivers is preferred. Key Expectations Within 12 Months: Effectively screen, assess, and onboard patients into The program. Maintain and manage an active patient caseload. Serve as a resource and support partner to team members to ensure high-quality service for the program population. Team Culture: Highly collaborative and supportive, with a focus on sharing resources and learning together. Flexible and adaptable as workflows evolve within this growing program. Deep compassion for patients and caregivers, keeping their needs at the center of the work. Onboarding Experience: Cerner registration training. Video modules covering CMS GUIDE Care model and dementia education. Independent review of onboarding materials. Shadowing of team members to learn operational tasks. Progression from shadowing → leading with support → leading independently.

Requirements

  • Bachelor’s degree or equivalent years of relevant experience.
  • 0–3 years of experience
  • Ability to work in a dynamic and complex business environment.
  • Demonstrated ability to manage multiple tasks, events, or projects simultaneously.
  • Strong communication and interpersonal skills
  • Effective organization and time-management abilities to balance outreach and patient management tasks.
  • Comfort working in a newer, evolving program with flexibility and a learning mindset.
  • Must reside in Indiana

Responsibilities

  • Serve as a bridge between the community and healthcare, government, and social service systems.
  • Assist individuals, families, and groups in accessing resources; provide informal counseling, support, and follow-up.
  • Coordinate frequently with CMS GUIDE Model team members and IU Health providers on behalf of patients.
  • Support community outreach strategies, initiatives, and events that meet identified community needs.
  • Effectively screen, assess, and onboard patients into The program.
  • Maintain and manage an active patient caseload.
  • Serve as a resource and support partner to team members to ensure high-quality service for the program population.
