About The Position

Provides direct services to individuals regarding concrete services and entitlements; assists with some aspects of discharge planning. Provides information to both “walk-in” patients and telephone inquiries. Oversees special workshops and socialization groups.

Requirements

  • Bachelor of Social Work required, or equivalent combination of education and related experience.
  • 0-1 years of relevant experience, required.

Nice To Haves

  • 3-5 years of relevant experience, preferred.

Responsibilities

  • Assists with the implementation of appropriate PAWH screenings and provide results to appropriate clinical staff.
  • Enroll eligible individuals into PAWH program.
  • Works with multidisciplinary MCM team to develop individualized service plan for PAWH patients.
  • Coordinates and monitors individualized service plan deliverables.
  • Assists with PAWH appointment adherence through coordination with Home Care, lab services and other ancillary support services to enhance retention.
  • Maintains appropriate HIPAA releases to contact designated familial/emergency contacts, support networks and CBOs.
  • Link PAWH enrolled individuals to referred providers, clinical services, Health Home, Insurance Navigation, support, and educational groups.
  • Assists care managers with cases requiring emergency intervention; discusses, with the social worker, care plan and means to facilitate patients’ goals.
  • Identifies unmet biopsychosocial needs and under the supervision of coordinator; assists patients to meet those needs effectively.
  • Maintains documentation in the patient chart in accordance with department requirements; prepares and submits reports as required.
  • Maintains related records and charts progress notes; documents the nature and extent of interventions made; makes copy of record for the Unit file.
  • Participates in interdisciplinary team meetings for discussion and implementation of plans of care.
  • Arranges patient transportation for program attendance and special trips or events; communicates with transport -company to arrange service.
  • Maintains documentation of case activity in the patient chart in accordance with hospital requirements.
  • Maintains current available treatment resource referrals and appropriate utilization.
  • Establishes and maintains professional relationships with patients and families.
