About The Position

This is a professional supervisory position for the Assisted and Residential Living facilities within Long-Term Care Campuses. It entails the supervision and management of assisted and residential living programs. Employees in this role enjoy significant freedom in planning, innovating, and managing program services, all under the guidance of the Administrator or Director of Long-Term Care Services. General hours of worked are 8:00 am - 4:30 pm at The Evergreens located on the Hillview Health Care Campus.

Requirements

  • Bachelor's degree in health care or human services fields such as nursing, recreational therapy, occupational therapy, sociology, social work, and psychology. A combination of relevant education and experience may be considered.
  • Two years of professional leadership/supervisory experience.
  • Must pass criminal record check as required by the Wisconsin Caregivers Law.
  • Employees driving La Crosse County vehicles must have and maintain a valid Driver's License and a good driving record.

Nice To Haves

  • Preferred one year of full-time experience in Wisconsin based Assisted Living CBRF.

Responsibilities

  • Oversees the day-to-day operation and function of all assigned programs. Directs specifications when appropriate. Works with teams and other departments to problem-solve complex care management issues.
  • Coordinates referrals and admissions to assisted living and residential programs in consultation with other staff. Facilitates the development of Resident Individualized Service Plans. Monitors and revises plans, as needed, in consultation with other community staff. Coordinates discharge planning in collaboration with critical stakeholders.
  • Negotiates service rates, reviews contracts, and modifies systems to meet contractual agreements. Supports the collections process to ensure timely payments per policy and procedure.
  • Ensures compliance with Assisted Living and Residential Living regulations. Responsible for developing and implementing a plan of correction as required.
  • Provides input during the annual budget development in collaboration with the Finance Department. Manages operational costs by assuring appropriate use of personnel resources and use of supplies and equipment. Determines the need for additional or replacement equipment.
  • Collaborates with the Director of Nursing to complete medical records and other resident information audits to ensure program regulatory compliance.
  • Investigates all allegations of misconduct, including abuse, neglect, misappropriation, and injuries of unknown origin. Review results with administration and provide a resolution to the complainant.
  • Hires, trains, supervises and evaluates all assigned staff while providing ongoing training, coaching, and support across all program areas. Completes performance evaluations and conducts disciplinary actions with supporting documentation.
  • Develops training curriculum for assigned staff in consultation with the Staff Development Coordinator.
  • Develops staffing patterns to meet the needs of the assisted and residential living programs. Supervises the development of daily schedules to ensure adequate staffing. May provide coverage on living areas.
  • Maintains and participates in an on-call schedule to ensure off hours support.
  • Develops and updates policies and procedures.
  • Acts as liaison with internal and external stakeholders to deliver quality services to residents.
  • Supports the Nursing Department in the delegation of nursing tasks and the coordination of clinical care for residents.
  • Supports all aspects of person-centered care. Respect and acknowledge the individuality and uniqueness of residents.
  • Attends mandatory in-service training.
  • Maintains residents' rights and confidentiality of resident information; reports all suspected cases of resident abuse to Administration.
  • Works safely and follows safety practices and standards of the campus; responsible for reporting or correcting any existing or potential safety or accident hazard.
  • The employee is expected to report to work when scheduled, with minimum use of sick leave, to build up sick leave balance for income protection for unexpected illness.
  • In this public service position, employee is required to be courteous, cooperative and respectful at all times, with the public and residents. Also establishes and maintains a courteous, cooperative and respectful working relationship with other employees, supervisors and public officials.
  • Represents the organization on a variety of internal and external teams.
  • May accompany and transport residents to worksite, appointments, and community events, which requires employees to have a good driving record.
