About The Position

The Clinical Director of Patient Services - Assisted Living oversees all aspects of resident care within an assisted living community. This role ensures high-quality clinical services, regulatory compliance, and effective coordination between nursing staff, residents, families, and external healthcare partners. The Clinical Director of Patient Services provides leadership to care teams, manages assessments and care plans, and supports a safe and supportive environment that promotes resident independence and well-being.

Requirements

  • New York Registered Nurse (RN) License Required.
  • 3–5 years of experience in assisted living, long-term care, or a related healthcare setting preferred.
  • Prior supervisory or management experience preferred.
  • Strong understanding of assisted living regulations and best practices.
  • Excellent leadership, communication, and interpersonal skills.
  • Ability to manage multiple priorities and make sound clinical decisions.
  • Strong organizational and documentation skills.
  • Compassionate approach to serving older adults and their families.

Nice To Haves

  • Bachelor’s degree in nursing, Healthcare Administration, Gerontology, or related field preferred.

Responsibilities

  • Oversee all clinical and personal care services provided to residents.
  • Conduct or supervise initial and ongoing resident assessments.
  • Develop, review, and update individualized care plans.
  • Ensure timely response to changes in residents’ health conditions.
  • Coordinate care needs with physicians, therapists, and other healthcare professionals.
  • Recruit, train, schedule, and supervise nursing and caregiving staff.
  • Conduct performance evaluations and provide ongoing coaching.
  • Ensure staff compliance with policies, procedures, and resident care standards.
  • Facilitate ongoing training in areas such as medication management, safety, and resident rights.
  • Ensure adherence to state and federal assisted living regulations.
  • Maintain accurate documentation and resident records.
  • Conduct quality assurance audits and implement corrective actions as needed.
  • Prepare for and support state inspections and license renewals.
  • Serve as a primary contact for resident and family concerns or questions.
  • Communicate changes in resident condition or care plans promptly.
  • Promote a positive and supportive environment through regular interaction and relationship-building.
  • Oversee medication management programs and ensure safe administration.
  • Collaborate with the Executive Director on budgeting and staffing needs.
  • Participate in community programs, safety committees, and interdisciplinary meetings.
  • Support emergency preparedness planning and response efforts.
