The Assisted Living Coordinator must exhibit genuine interest in residents, showing patience and understanding while working with residents, families, and visitors. This role involves hiring, training, supervising, and scheduling caregivers and med techs. The coordinator monitors resident care needs, alerts the Wellness Director to changes in condition, addresses resident and family concerns, and acts as a liaison with community providers. The person in this position is responsible for carrying out duties in accordance with current federal and state regulations and established company policies and procedures.

Requirements

  • Demonstration of Core Values: Love, Kindness, Honesty, Goodness, Fairness, Respect.
  • Successful completion of State approved Medication Administration training.
  • Current first aid certificate as required.
  • Minimum of 2 years of health-related experience.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, procurement manuals.
  • Possess strong communication skills.

  • Supervisory or management experience in long-term care or assisted living is preferred.

Responsibilities

  • Hires, trains, supervises, and schedules caregivers and med techs.
  • Monitors resident care needs and alerts the Wellness Director to changes in condition.
  • Addresses resident and family concerns and acts as a liaison with community providers.
  • Manages staffing needs by interviewing, hiring, training, supervising, and scheduling care managers and MCM coverage.
  • Assists the Business Office Manager with new hire paperwork, tracking attendance logs, monitoring time sheets, performing employee evaluations, and staying within the staffing budget.
  • Identifies staff educational needs, schedules and coordinates new hire orientation, weekly mini-modules, and mandatory in-service meetings.
  • Ensures care staff is educated about resident needs and knowledgeable of resident service plans and task/assignment sheets.
  • Reports symptoms and changes in resident condition to the Wellness Director.
  • Maintains accurate records of resident conditions, documents in the Daily Log, and reports all changes to the Wellness Director.
  • Ensures resident care is provided by weekly monitoring of resident suites and daily walk-throughs of assisted living areas.
  • Ensures all infection control guidelines are met and monitors availability of gloves and protective supplies.
  • Possesses the ability to problem-solve effectively and manage various situations according to MorningStar standards and guidelines.
  • Prepares and administers medications according to procedure.
  • Works with the Wellness Director to develop, maintain, and update resident negotiated service plans and assessments.
  • Schedules and conducts change of condition assessments and bi-annual negotiated service plan meetings.
  • Maintains ongoing communication with residents and families regarding care needs and assists with counseling as needed.
  • Responsible for regular resident suite checks and prompt answering of emergency alert systems.
  • Reports incidents of suspected resident abuse immediately and completes incident reports within 24 hours.
  • Completes Incident Tracker monthly and works with the Wellness Director on addendums for interventions and reductions in incidents.
  • Reviews incidents at the QA/safety committee meeting monthly.
  • Assures resident weights and vital signs are taken correctly and recorded at specific times.
  • Directs caregivers to participate in taking residents to activities and to spend additional one-on-one time with residents unable to attend.
