About The Position

The Assisted Living Coordinator coordinates and addresses the health concerns of Assisted Living residents, including resident assessments, medication and pharmacy management, activities of daily living, and personal care needs. The coordinator collaborates with nursing leadership on day-to-day clinical operations across the campus and serves as the lead nurse for assisted living residents. The role also participates in the on-call rotation as designated by the Director of Nursing.

Requirements

• Active, unencumbered LPN license in the state.
• CPR certification.

Responsibilities

• Collaborate with the Director of Nursing and Assistant Director of Nursing on day-to-day clinical services for the campus.
• Ensure each resident’s pre-admission screen is completed before move-in.
• Direct residents’ Lifestyle and Care Needs Assessments to determine initial and ongoing medical and personal care requirements.
• Assist residents with activities of daily living, including bathing, dressing, grooming, and dining, and administer medications.
• Lead and assist in developing service plans with residents and families, and communicate plans to caregivers.
• Facilitate Assisted Living Resident Meetings and follow up promptly on feedback and concerns.
• Work as the lead nurse for assisted living residents and participate in the on-call rotation as designated by the Director of Nursing.
