Under direction of the Social Worker Supervisor, the Extra Help Assistant Social Worker/Social Worker will complete In Home Supportive Services assessments and re-evaluations for eligible recipients. The Assistant Social Worker/Social Worker will conduct in-home assessments and reassessments to determine client eligibility and level of need for IHSS services. They will interview clients, family members, and caregivers to gather information related to functional abilities and living conditions. They will utilize state-mandated assessment tools (e.g., SOC 295, Functional Index Rankings) and develop individualized service plans and authorize appropriate hours of care. They will monitor and reassess client conditions to ensure ongoing eligibility and adequacy of services; collaborate with healthcare providers, community organizations, and other service agencies; maintain accurate case records in state and county databases (e.g., CMIPS); and respond to reports of suspected abuse or neglect, making referrals to Adult Protective Services as appropriate.
