Assistant Discharge Planner - Hudson Valley – Full Time - Day Shift
About The Position

Explore this opportunity: Assist with facilitating successful discharge plans which are critical to LOS reduction and patient flow. Locate resources, rapidly respond to requests by Care Management, Social Work, and Unit Staff. Provide a high level of customer service to patients when preparing for discharge. Make regular rounds of all patients to assure that assigned treatment plans are being implemented as determined by the treatment team. Work closely with Care Management and Social Work to assure proper and timely discharges of patients. Ensure communication is relayed on discharge plan details to the interdisciplinary treatment team.
Requirements
- High school diploma or GED.
- At least one year of experience in a healthcare setting is required. Volunteer and internship time is acceptable.
- Strong interpersonal skills, willingness to learn and ability to work in a fast paced and changing environment.
- Knowledge of Microsoft Office products and general computer literacy.
- Excellent communication skills, both written and oral.
- Detail oriented, good organizational skills and ability to multi-task.
- Ability to work independently.
