About The Position

The Discharge Planner coordinates and implements post hospital care by evaluating and assessing the patients' needs, financial status/insurance coverage and community resources available. Discharge planning is coordinated with physicians, Nursing, Case Managers/Care Coordinators and the patients along with their families/significant others who will be involved in their post-hospital care.

Requirements

• Mature individual, capable of being objective while empathetic and supportive of patients and family's needs.
• Able to communicate verbally with all levels of society.
• Able to read, write, and speak English fluently.
• Able to communicate effectively in Spanish.
• Ability to work with individuals of all ages, races, and religious backgrounds.
• Good organizational skills.
• Able to remain calm in stressful situations.
• Basic computer skills.
• Basic knowledge of community resources.
• High school graduate with three to five years' experience in an acute care setting and/or Social Work-related field preferred.

Nice To Haves

• Associate of Art or Associate in Science Degree preferred.
