About The Position

White Blossom Care Center is a premier 153-bed skilled nursing and short-term rehabilitation facility featuring the largest rehabilitation gym in the Bay Area. We operate with a dedicated in-house therapy team, offering extensive clinical resources, advanced modalities, and a supportive environment focused on functional recovery. We are seeking a motivated and compassionate clinical leader to serve as both an Assistant Director of Rehab (ADOR) and an Occupational Therapist (OTR). This role involves providing excellent patient care and assisting the Director of Rehab in managing daily operations, leading team meetings, tracking compliance, and mentoring staff. It's an ideal opportunity for an experienced OTR to move into clinical leadership within a well-resourced facility.

Requirements

Graduate of an accredited Occupational Therapy program.
Active and unencumbered Occupational Therapist (OTR) license issued by the California Board of Occupational Therapy.
Minimum of 2–3 years of clinical experience in a skilled nursing facility (SNF) or post-acute care setting preferred.
Strong understanding of Medicare PDPM guidelines, clinical documentation software, and team coordination.

Responsibilities

Partner with the Director of Rehab to supervise, schedule, and support therapists and COTAs.
Assist with utilization reviews, daily department auditing, clinical compliance monitoring, and care conferences.
Perform comprehensive evaluations and deliver evidence-based occupational therapy interventions.
Oversee a diverse clinical caseload including Orthopedic, Neurological, Cardiac, and Vascular rehabilitation.
Ensure timely and precise completion of evaluations, daily notes, and recertifications across the department.
Coordinate durable medical equipment (DME) ordering and provide family/caregiver training for safe discharges.
