About The Position

The Assistant Director of Reentry & CalAIM Services (ADRCS) provides leadership and operational oversight of Enhanced Care Management (ECM), Community Health Worker (CHW), and Community Supports (CS) programs. This role requires strong knowledge of ECM, CHW, and CS service delivery, medical billing practices, and experience supervising care management staff. The Associate Director ensures the effective day-to-day functioning of services, supports high-quality care coordination, and promotes compliance with program requirements and organizational standards.

Requirements

  • Bachelors degree in social work or related field
  • At least three (3) years of supervisory or program leadership experience.
  • Demonstrated experience with ECM, CHW, and CS service delivery, care planning, documentation and quality assurance
  • Strong knowledge of CalAIM, DHCS requirements, and Medi-Cal billing practices
  • Strong commitment to client self determination and professional practice
  • Working knowledge of the principles of confidentiality and HIPAA
  • Experience supervising multidisciplinary teams, including care managers or housing support staff
  • Knowledge of housing systems, homelessness services, and justice impacted populations
  • Strong understanding of documentation standards, compliance, and data reporting requirements
  • Excellent leadership, organizational, and communication skills
  • Valid California Driver’s License and auto insurance
  • Ability to pass a criminal background check
  • Excellent verbal and written communications skills required
  • Experience working with diverse populations, particularly justice impacted adults and those with economic disadvantages
  • Strong organizational, time management and problem-solving skills
  • Experience with data entry and proficiency with computers including Microsoft Office Suite: Word, Excel, Power Point, Publisher, Outlook, and Internet, and electronic database systems.
  • Demonstrated understanding of community resources and how to access them
  • Ability to work independently, be flexible and dependable, and demonstrate initiative and creativity

Responsibilities

  • Manage the day-to-day operations of ECM, CHW and CS services, including care navigation, Housing Transition Navigation, Housing Deposits, and Housing Tenancy and Sustaining Services
  • Ensure all services are delivered in accordance with DHCS, CalAIM, and organizational policies and procedures
  • Oversee implementation of Housing First, trauma-informed care, motivational interviewing, and progressive engagement models in service delivery
  • Monitor program quality, capacity, staffing levels, and service utilization; adjust operations as needed to meet demand and compliance requirements
  • Supervise lead care managers and housing support specialists
  • Provide ongoing coaching, case consultation, and performance management
  • Facilitate regular team meetings, including multidisciplinary case conferences and care coordination discussions
  • Ensure staff adherence to required training, documentation standards, and service protocols
  • Oversee development and implementation of individualized care plans and housing support plans based on member needs, goals, and preferences
  • Ensure effective coordination of care across physical health, behavioral health, housing, and social service systems
  • Support high-acuity case consultations and guide staff in addressing complex member needs
  • Ensure timely outreach and engagement, including in-field engagement and meeting members where they live or seek services
  • Ensure accurate and timely documentation in electronic health records, including assessments, care plans, progress notes, and consent forms
  • Oversee compliance with HIPAA, data-sharing requirements, and consent/authorization protocols
  • Support and monitor medical billing and encounter data submission processes for ECM and CS services
  • Ensure program adherence to authorization, reauthorization, and eligibility determination processes
  • Oversee housing-related service delivery, including: Tenant screening, housing assessments, and individualized housing support plans, Housing search, placement, and landlord engagement, Tenancy stabilization, eviction prevention, and crisis intervention planning
  • Ensure staff follow required service structures (e.g., tiered landlord engagement, crisis escalation protocols)
  • Strengthen partnerships with housing authorities, landlords, and community-based organizations
  • Oversee program monitoring, data collection, and reporting, including quarterly and required DHCS/MCP reporting
  • Track performance metrics and implement continuous quality improvement strategies
  • Ensure readiness for audits, reviews, and contract compliance monitoring
  • Work in partnership with the contracted clinical consultant to ensure clinical integrity and appropriate service delivery
  • Collaborate with Managed Care Plans (MCPs), county agencies, correctional systems, and community partners
  • Maintain strong coordination with internal leadership and cross-functional teams
