About The Position

The Assistant Director of Ongoing Case Management provides operational and compliance oversight and people leadership for defined CMA service segments: Children–Young Adult (ages 0–26), Adult (ages 27–59), and Aging Adults (ages 60+). This role ensures services are delivered timely and consistently and in alignment with Colorado Department of Health Care Policy and Financing (HCPF) requirements. This position supports a scalable, team-based operating model focused on system oversight, trend monitoring, risk mitigation, and leader development, and is critical to audit readiness, workforce sustainability, and long-term organizational scalability. This role establishes operational consistency and service excellence across El Paso, Pueblo, Park, and Teller counties to ensure eligible members and their families receive high-quality support tailored to their needs.

Requirements

  • Proven experience managing stakeholder relationships and leading cross-functional teams.
  • In-depth knowledge of Colorado regulations governing CMAs, including Medicaid waivers and other funding streams.
  • Strong knowledge and familiarity with Colorado’s CMA system and its role in Long Term Services and Supports.
  • Demonstrated project management tools and methodologies.
  • Knowledge of training, design theory and delivery best practices.
  • Excellent communication, leadership, strategic planning, and organizational skills.
  • Excellent organizational, problem-solving, and decision-making skills.
  • Exceptional interpersonal and communication abilities.
  • Minimum: Bachelor’s degree in social work, human services, public administration, healthcare management, quality management, or a related field.
  • Minimum of 4 years of leadership experience in program management within the IDD, non-IDD, Children’s Waivers, or human services sector.
  • Computers/tablets, Printers, Copy Machines, Personal Vehicle, Cell phones/telephone, Cloud based systems and software, etc.

  • 3+ years’ experience in supervisory and budget management preferably within a human service or health care organization.
  • Alternate preferred qualifications include 5+years experience in supervisory and budget management within a human service of health care organization.
  • Thorough and specific knowledge and application in all waivers to include children’s, adult and mature adult waivers preferred.

Responsibilities

  • Oversee CMA operations for an assigned age cohort across services.
  • Monitor trends related to timeliness, escalations, workload, audit findings, and compliance risk.
  • Participate in on-call and regional rotation structures to ensure leadership coverage.
  • Provide escalation support to Managers and Level 6 Escalation Coordinators.
  • Represent CMA leadership in HCPF stabilization meetings as assigned.
  • Oversee Intake operations to ensure regulatory compliance and high-quality member engagement, and to coordinate effectively with other teams.
  • Ensure alignment with HCPF rules, waiver requirements, and TRE policies.
  • Partner with Quality & Compliance on policy development and implementation.
  • Oversee consistent audit standards and corrective actions.
  • Represent the CMA in appeals, grievances, and high-risk compliance matters.
  • Directly supervise Managers and support consistent leadership practices.
  • Hold weekly connections with Managers and bi-monthly 1:1s focused on coaching and development.
  • Support succession planning across Supervisor, Manager, and other case management roles.
  • Model inclusive leadership aligned with TRE values.
  • Provide strategic guidance to Supervisors Managers and Coordinators to ensure compliance with state and federal requirements while meeting operational goals across TRE.
  • Align CMA operations and leadership practices with the overall vision and direction of the CMA.
  • Support effective communication between CMA staff, leadership, and other organizational departments.
  • Serve as a cross-department liaison to support timely issue resolution and alignment with shared strategic goals.
  • Develop and maintain strong collaborative relationships with external stakeholders including service providers state agencies advocacy groups and community organizations.
  • Represent the CMA in public forums, partner meetings, and strategic planning initiatives, as assigned.
  • Monitor staffing models, member volume, and capacity.
  • Partner with Finance and People Services on workforce planning training and budgeting.
  • Contribute to strategic planning efforts including the development of operational goals performance targets and long term program improvements.
  • Support the design and implementation of new initiatives workflows and service models aligned with HCPF guidance and organizational priorities.
  • Use data and trends to inform training and workflow improvements.
  • Analyze operational trends and recommend strategies to improve intake efficiency and member outcomes.
  • Lead or support special projects as assigned by the Director of CMA.
  • Serve as liaison for specialty and cross-functional initiatives as assigned.
  • Participate in All-Staff and other TRE facilitated meetings and activities.
  • Participate and engage in training and development opportunities.
  • Other duties as assigned.
