Requirements

  • Must be a Licensed Practical Nurse or Registered Nurse, licensed to practice in the state.
  • Experience in Assisted Living, Home Health, Skilled Nursing and/or retirement center.
  • Experience and passion in working with seniors.
  • Experience in supervisory role and able to lead.
  • Able to perform tasks which may be physically demanding such as pushing, bending and lifting up to 60lbs.
  • Ability to maintain positive working relationships with residents, their families, peers and other staff members.

Responsibilities

  • Responsible for ensuring clinical services are provided in accordance with state regulations, accepted clinical standards, agency policy and standards.
  • Assist in the coordination and management of nursing and personal care attendant services provided to residents.

  • Provision of care and services to residents whose conditions are chronic and stable
  • Verifying residents’ medication regimen with physicians or heath care practitioners for residents who require supervision
  • Assist residents with activities of daily living, including bathing, dressing, eating, toileting, hygiene and mobility.
  • Aware and knowledgeable of the federal and state laws and regulations regarding assisted living/memory care and the regulations of the state’s nursing commission; ensures and maintains regulatory compliance with all such regulations.
  • Assisting in the coordination and management of care and services for residents through the following:
  • Collect and document objective and subjective data and observations for the pre-move in assessment, review of paperwork and contribute to initial service plan development.

  • Coordinate resident’s health care in collaboration with residents, families and outside health care agencies, including referral when indicated
  • Conducts regular Service Plan Reviews with resident families, including meetings with responsible parties and including appropriate staff whenever changes in care levels occur.
  • Coordinates and participates in initial job specific orientation and training through routine review of training documents, in accordance with policies and procedures
  • Coordinates and conducts continuing training and education on a regular basis, as identified through routine auditing
  • Reviews documentation and communication of Resident Care Services associates
  • Supervises assigned personnel in the delivery of nursing services and assistance with activities.
  • Provides delegation, in accordance with state regulations and Community policy.
  • Available after-hours for issues related to current residents’ needs; may be required to come into the Community or conduct offsite assessment at these times.
  • Maintains compliance with Community policies on Immunizations, Infection Control and Infectious Disease Management.

  • Assists the Resident Service Director in monthly Resident Care Service staffing schedule.
  • Available after-hours, on-call, related to Resident Care
  • In coordination with the Resident Service Director, provides weekly reports to Executive Director, regarding any issues associated with the provision of core services, the managed residential community or the ALSA, summaries of which shall be provided to the governing authority, in accordance with the schedule established by the governing authority.
  • Participates in other Resident Care Services and Community related duties, as assigned by the Executive Director
