About The Position

The Assistant Director of Incident Management (IM) oversees investigations of incidents consistent with CCANY’s Incident Management program and policies. Completes assigned projects and initiatives for the IM Department, such as compiling data relating to incidents and other IM-related needs. Actively utilizes systems including Incident Reporting Management Application (IRMA), CHOICES, and internal monitoring and reporting systems while adhering to regulatory guidelines. The Assistant Director of Incident Management also supervises the Incident Management Investigators.

Requirements

  • Associates degree required.
  • At least 5 years of experience required as a team leader and/or manager.
  • Strong organizational and project management skills and demonstrated attention to detail.
  • Ability to use discretion and maintain the confidentiality of information obtained in conduct of work.
  • Excellent interpersonal, public speaking, and written communication skills.
  • Strong documentation, analytical, and problem-solving skills is required.
  • Excellent working knowledge of Microsoft Word, Excel, Access, and PowerPoint.
  • Must possess a valid Driver’s License from New York, or a contiguous state (i.e., Connecticut, New Jersey, Pennsylvania, and Vermont OR Must have the ability to take ample public transportation to attend meetings in person in the community and in the office as needed.

Nice To Haves

  • Bachelor’s degree with majors in Human Services and Health Care Management fields is preferred.
  • Graduate degree (Master’s and/or Doctorate) preferred.
  • Three (3) years of job-related experience, specifically in OPWDD, DOH, Incident Management, Compliance and Policies, and Investigations is preferred.
  • Certified and Trained OPWDD Investigator preferred.
  • Experience working with Electronic Health Records.

Responsibilities

  • Serves as the primary contact on all Incidents.
  • Serves as CCANY’s Liaison with the OPWDD and DOH.
  • Solely responsible to ensure and adhere to yearly protocol, including annual review of IM program policies.
  • Understands and applies governmental laws and regulatory rules; Keeps current on trends.
  • Applies sound investigative techniques and practices, oversees investigations completed by the Incident Investigators to ensure compliance with regulations, laws, and CCANY program policies, and procedures.
  • Makes recommendations for resolution of issues.
  • Tracks that corrective action/follow up have been completed, timely and as prescribed.
  • Maintains all documents, files, and records ensuring they are complete and secure.
  • Utilizes required databases to enter information timely, accurately, completely.
  • Collaborates with the Training Department to deliver training for employees’ areas of responsibility, as required.
  • Compiles statistics, performs analysis to determine trends and generate appropriate reports, monthly and annually, or as requested.
  • Protects the rights of individuals served and makes sure informed consent measures are adhered to.
  • Consults with employees at all levels on issues pertaining to Incident Management and decisions impacting the outcomes of the incident.
  • Chair internal Incident Review committee and reports on committee activity quarterly.
  • Provides professional presentations, training sessions, reports and updates to internal committees and executive leaders.
  • Prepares letters, memoranda, statements, narratives, statistical reports, and agenda as requested or according to established procedures, including when redaction is required/recommended.
  • Manages and implements projects assigned by the Director of Compliance and Incident Management; Privacy Officer and/or the Vice President of Quality and Compliance & Risk Management.
  • Participates in special projects and all other job-related tasks as assigned by the Director of Compliance and Incident Management and/or the Vice President of Quality and Compliance & Risk Management.
  • Performs other duties, as assigned.
  • Supervises a team of Incident Investigators.
  • Ensures coverage of the Incident Management reporting system at all times.
