About The Position

Hamilton-Madison House’s Caregiver Support Program serves informal/unpaid caregivers of older adults and their loved ones, throughout New York City’s five boroughs, with an emphasis on the linguistic and cultural competence needs of Asian and Southeast Asian families. The Assistant Director (AD) will be responsible for the supervision of 7 HMH program care managers. The AD will supervise the Intake Coordinator. Upon completion of intake, provide a review process and assign new care giver clients to an appropriate care manager. The Assistant Director will utilize Vive and other tracking systems to maintain timelines of all requirements. Assists the director with oversight of the distribution of services to clients; and completion and submission of all reports to the NYC Department for the Aging, the funding source for this program. This position reports to the Program Director of Manhattan Case Management Services.

Requirements

  • Master's degree in social work or related field required or Bachelor's degree with relevant Care Management Experience
  • 3 years of supervisory experience preferred
  • SIFI Certification Preferred
  • Excellent clinical social work skills including client engagement, assessment, and the biological, psychological, and social aspects of aging and the impact of disabilities and illness on aging and the family
  • Excellent written and verbal communication skills
  • Strong management skills
  • Knowledge of New York City and New York State and federal benefits and entitlements
  • Ability to develop collaborative relationships with professional staff and community groups
  • Excellent verbal and written communication skills
  • Engaging and Strong interpersonal skills demonstrative of empathy and support
  • Strong computer literacy and Microsoft Windows and Microsoft Office
  • Strong understanding of client data base and experience with Vive preferred

Responsibilities

  • Provide oversight and training to project team of 7, Care Managers (MSW, BSW, Interns).
  • Provide individual supervision, case conferences, and interdisciplinary team meetings.
  • Responsible to prepare and review all intakes and assign to trained staff, especially those with language skills and cultural knowledge or experience with special populations, i.e., Older Adults raising adult children with intellectual challenges.
  • Ensure that timelines for assessments, reassessments, and all deliverables are met by the main team.
  • Coordinate with the Program Aide in reviewing Vive, Excel ticklers to ensure all deadlines and requirements are made.
  • Review and approve assessments, care plans of assigned HMH Care Managers.
  • Review and approve teams' request for supplemental supplies and respite services.
  • Assist in the development of caregiver training workshops and support groups for caregivers in their role and help them maintain their own health and wellbeing.
  • Assist with planning and implementation of the program's annual Caregiver Appreciation Event.
  • Assist with annual client survey and review and follow-up on areas for improvement.
  • Work with colleagues within the Hamilton-Madison House Senior Services Department to ensure that available resources are maximized, and program efforts are integrated within the department.
  • May require you to work flexible hours to meet the needs of the working caregivers.
