About The Position

Reporting to the Home Care Manager, the Client Case Coordinator is responsible for the assessment of clients, coordination and evaluation of Home Care services, and case management of Region residents. The Client Case Coordinator will ensure that the delivery of home care and case management services is consistent with the policies, vision, mission, and philosophy of the Saskatoon Health Region. Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement.

Requirements

• Bachelor of Science in Nursing (BScN)
• Bachelor of Social Work is acceptable
• Nursing diploma is acceptable
• Psychiatric Nursing diploma is acceptable
• Bachelor of Psychiatric Nursing is acceptable
• Practical Nursing diploma with 4 years experience is acceptable
• Advanced - Negotiation skills
• Advanced - Communication skills
• Advanced - Mediation skills
• Advanced - Problem solving skills
• Advanced - Decision making skills
• Ability to work independently
• Ability to work and communicate in a cross cultural setting
• Knowledge in the SHA, community resources and applicable programming
• Ability to work as a member of a multi-disciplinary team
• Registered/eligible with the Saskatchewan Association of Social Workers (SASW)
• Must upload all relevant documents (ex: certificates, diplomas, proof of enrollment, licenses and/or memberships) relating to the qualifications of the position.
• Must bring a valid Criminal Record Check (CRC) to the interview; it must be dated within six (6) months and include a vulnerable sector search.
