About The Position

Vanderbilt University Medical Center (VUMC) is seeking an Assessment Specialist for the Psych Access Center. This role involves performing clinical crisis assessments, telephone triage, referral coordination, and disposition planning for psychiatric patients of all ages. The specialist will document clinical concerns accurately in the patient record and possess the mental health knowledge to refer patients to the appropriate level of care within the behavioral health continuum. LIC-Licensed Clinical Social Worker - Tennessee
LIC-Licensed Professional Counselor-Mental Health Service Provider - Licensure-Others
Relevant Work Experience
Master's degree
2 years of relevant work experience

Responsibilities

Completes Crisis Assessment independently via in person and/or tele psych
Documents accurately in patient chart and has the ability to update the team during shift changes
Facilitates Admissions / assessments with Physician partners
Performs crisis intervention
Telephone Triage Activities
Contributes to the organization by actively participating in process improvement initiatives and shared decision making
