About The Position

Vanderbilt University Medical Center (VUMC) is seeking an Assessment Specialist to join their team in Nashville, Tennessee. VUMC is a leading institution focused on advancing health and wellness through patient care, education, and research. The Assessment Specialist will perform clinical crisis assessments, telephone triage in the psych access center, referral coordination, and disposition planning for psychiatric patients of all ages. This role requires accurate documentation in the patient record and a strong understanding of mental health to refer patients to the appropriate level of care within the behavioral health continuum. LIC-Licensed Clinical Social Worker - Tennessee
LIC-Licensed Professional Counselor-Mental Health Service Provider - Licensure-Others
Relevant Work Experience
Master's degree
2 years of relevant work experience

Patient Documentation (Intermediate): The ability to receive and record patient information.

Responsibilities

Completes Crisis Assessment independently via in person and/or tele psych
Documents accurately in patient chart and has the ability to update the team during shift changes
Facilitates Admissions / assessments with Physician partners
Performs crisis intervention
Telephone Triage Activities
Contributes to the organization by actively participating in process improvement initiatives and shared decision making
