About The Position

Vanderbilt University Medical Center (VUMC) is seeking an Assessment Specialist to join their team in Nashville, Tennessee. VUMC is a leading institution focused on advancing health and wellness through patient care, education, and research. The Assessment Specialist will perform clinical crisis assessments, telephone triage in the psych access center, referral coordination, and disposition planning for psychiatric patients of all ages. This role requires accurate documentation in the patient record and a strong understanding of mental health to refer patients to the appropriate level of care within the behavioral health continuum. Requirements

  • LIC-Licensed Clinical Social Worker - Tennessee
  • LIC-Licensed Professional Counselor-Mental Health Service Provider - Licensure-Others
  • Relevant Work Experience
  • Master's degree
  • 2 years of relevant work experience

  • Crisis Intervention (Advanced): Demonstrates the uppermost levels of expertise in applying crisis intervention in challenging and complex situations. Often takes a lead role in highly pressurized situations when there is a need to achieve results. Consistently delivers results in a creative and imaginative manner while setting a standard for others. Contributes in the development of best practices for the organization.
  • Training (Intermediate): Has delivered a program in its entirety. Managed the delivery of a program by a team of instructors, assuring attention to detail. Demonstrated the ability to deal with adversity and disruption during delivery. Established class leadership and credibility. Demonstrated the ability to give clear, concise instructions in support of participant activities.
  • Patient Documentation (Intermediate): The ability to receive and record patient information.
  • Contract Negotiations (Intermediate): Demonstrates an understanding of negotiating skills such as building trust, gaining commitment, managing the opposition and maintaining goodwill. Realizes the need for mutual concessions and accommodations and negotiates with others for resources, actions or buy-in to maximize collaborative outcomes.

Responsibilities

  • Completes Crisis Assessment independently via in person and/or tele psych
  • Documents accurately in patient chart and has the ability to update the team during shift changes
  • Facilitates Admissions / assessments with Physician partners
  • Performs crisis intervention
  • Telephone Triage Activities
  • Contributes to the organization by actively participating in process improvement initiatives and shared decision making
