We are currently seeking a Assessment Specialist Supervisor to join our Iowa Clinical Department. Requirements

  • Active License as RN, LSW or LPN.
  • 12 months experience in home and community-based services within the past 5 years.
  • Ability to instruct, counsel, and evaluate subordinate professional staff; effective oral and written communication skills with attention to detail in assessment and in documentation.
  • Excellent organizational skills with the ability to prioritize projects and be attentive to detail.
  • Effective interpersonal skills with respect for others, including consumers, co-workers and other professionals within a diverse work environment.
  • Demonstrates strong critical thinking and problem solving skills and flexibility in relationship to job requirements.
  • Computer skills sufficient to enter and process consumer information, report preparation, and use CareStar’s website and email, and familiarity with suite of Microsoft Office programs.
  • Valid driver’s license and car insurance, as required by State law, and reliable transportation to drive to other CareStar offices, as business requires.
  • Adheres to the CareStar Rule in performance of job responsibilities.

Responsibilities

  • Maintains confidentially as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offence.
  • Follows the acceptable use policy while using any information systems owned or controlled by CareStar.
  • Directly supervises, manages, and oversees staff, including hiring, firing, and providing performance evaluations, and individual development goals and objectives.
  • Ensures functions of the department are in compliance with appropriate program requirements, CareStar policies and guidelines, and contract standards.
  • Monitors intake inquiries and requests; ensures appropriate information and referral services are provided in a timely manner.
  • Oversees the assignment of initial eligibility assessments and other clinical work to appropriate staff and new individuals to a Clinical Supervisor.
  • Monitors and tracks applicant status until a final determination is made.
  • Responds to inquiries and provides supporting documentation regarding cases as necessary.
  • Monitors the initial enrollment process for potential high-risk situations and notifies management as appropriate.
  • Participates on agency committees and performs quality improvement activities as needed.
  • Serves on work groups, community groups, committees, or other task forces and attends meetings as requested.
  • Participates in sales presentations, RFPs, RFIs and other proposals.
  • Performs other projects and duties as assigned.
