About The Position

WellBridge Healthcare Fort Worth, part of Lifepoint Health, is seeking an Assessment Specialist RN for a PRN position. This role is crucial in identifying patients appropriate for admission to the Behavioral Health Unit and conducting crisis assessments. The position involves direct and indirect patient care through the nursing process, including developing care plans in coordination with patients, families, and interdisciplinary staff, and communicating changes in patient condition to relevant stakeholders.

Requirements

• Current state RN license
• Bachelor’s degree from an accredited nursing school
• BCLS certification
• Minimum of 2 years’ experience in a psychiatric setting

Nice To Haves

• ACLS preferred
• Experience in crisis counseling and crisis evaluations preferred

Responsibilities

• Working with internal and external customers to identify patients appropriate for admission to the Behavioral Health Unit.
• Conducting a crisis assessment as identified in the essential job specific functions.
• Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation.
• Develops nursing care plan in coordination with patient, family and interdisciplinary staff as necessary.
• Communicates changes in patient’s clinical condition with Physicians, Nursing Supervisor/Manager and Director of Intake and Admission as appropriate.
