Flexible scheduling for many roles Position Summary Provide intensive individual, group, and crisis counseling case management services to CCRS/TASC clients. Interviews referred individuals and screens referrals to determine eligibility into CCRS/TASC Program and Court Clinic’s Behavioral groups. Develops, implements, and monitors treatment plans for everyone on caseload; coordinates and consults with others to develop treatment plans; provides assessments information. Provides monthly progress reports to Probation or Diversion Officers by the 5 th working day of the month. Provides consultation for supervisors; prepares case material for discussion with consultant. Attends case conference meetings with supervisor to discuss, review, and analyze caseload for evaluation of services delivered. Facilitates Substance Use Disorder treatment groups, as needed. Demonstrates regular and predictable attendance. Develops and maintains daily and cumulative records for everyone served in a timely manner; prepares related correspondence and reports; attends training. Provides referral to treatment provider within 7 working days of client’s acceptance to TASC; works closely with criminal justice, treatment personnel, and community resources to coordinate treatment/counseling. Establishes and maintains positive relationships with client, staff, treatment providers, probation officers, outside agencies, and community. Make court appearances. Other duties as assigned.
