About The Position

This role involves collaborating in the establishment of the home health plan of care, coordinating and implementing care delivery, and anticipating patient care needs across the continuum. The position requires analyzing comprehensive information pertinent to the patient's care and management to determine nursing diagnosis. The nurse will continually utilize the nursing process to evaluate patient's progress towards goals and apply appropriate interventions to ensure effective outcomes. Strategies to promote health and a safe and healthy environment will be employed. The responsibilities listed are a general overview, and additional duties may be assigned. Vanderbilt Home Care delivers high-quality, in-home care through licensed nurses and therapists who support patients in the comfort and independence of their own homes. Under physician direction, our team assists with daily living activities such as bathing, dressing, eating, medication management, and provides companionship and emotional support. Responsibilities
