About The Position

Vanderbilt Home Health is seeking an Assessment RN to join their team in Rutherford County. This role involves collaborating on home health plans of care, coordinating and implementing care delivery, and anticipating patient needs. The nurse will analyze patient information to determine nursing diagnoses, evaluate patient progress, and apply interventions for effective outcomes. The position emphasizes promoting health and a safe environment. The role involves approximately 36 hours per week, with assessment visits typically lasting around 2 hours each. Vanderbilt Home Care delivers high-quality, in-home care through licensed nurses and therapists who support patients in their homes, assisting with daily living activities, medication management, and providing emotional support. They serve patients across several counties, committed to safe, patient-centered care.

Requirements

  • LIC-Registered Nurse - Licensure-Others
  • Relevant Work Experience
  • Graduate of an approved discipline specific program
  • 1 year of relevant work experience

Responsibilities

  • Collaborates in establishing the home health plan of care, coordinates and implements care delivery while anticipating patient care needs across the continuum.
  • Analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis.
  • Continually utilizes the nursing process to evaluate patient's progress towards goals and applies appropriate interventions to ensure effective outcomes.
  • Employs strategies to promote health and a safe and healthy environment.
  • Conducts primary care patient interviews, assesses the safety of the home environment and performs a comprehensive physical assessment.
  • Recognizes problems, asks questions and contacts the patient's primary care provider.
