About The Position
Palm Point Behavioral Health (affiliated with Universal Health Services) is a 74-bed behavioral health hospital located in Titusville, Florida. Palm Point Behavioral Health treats adults, children, and adolescents suffering from a wide range of behavioral health diagnoses, including depression, anxiety, and substance abuse, in a modern and well-appointed facility. Palm Point Behavioral Health offers inpatient treatment options and serves communities in Brevard County and beyond. Titusville, located on Florida’s Space Coast, is approximately 30 miles north of historic Cocoa Beach and 40 miles east of Orlando’s famous theme parks. In Titusville, you can tour the Kennedy Space Center Visitor Complex, Merritt Island National Wildlife Refuge, and Canaveral National Seashore. Not only do you have the abundance of activities along the Atlantic shore, but Titusville also offers an ideal climate, close proximity to multiple dining and shopping options, and a very friendly community. Palm Point is currently recruiting for an Assessment/Referral Coordinator. The Assessment/Referral Coordinator is responsible for being able to perform all aspects of Palm Point Behavioral Hospital’s intake and admission process for both Inpatient and Outpatient Services, including efficient and effective handling of inquiry calls, knowledge of and communication with referral sources; providing hospital and program information to individuals, agencies, facilities, and professionals; making outside referrals, when appropriate; assessing and triaging an individual’s mental health status; making patient safety and level of care recommendations to the PPBH physician; identifying insurance benefits, pre-certification of patient care; completing all admission requirements; communicating patient safety concerns and pertinent patient information to unit nursing staff and documenting all necessary information in Med Series 4.
Requirements
- Licensed RN or Master's Degree from an accredited school in the Behavioral Sciences.
- RN, LCSW, LMHC, LMFT preferred.
- BLS required or obtained prior to direct patient care.
- 1-3 years of experience preferred in Florida psychiatric health care.
Responsibilities
- Responds to telephone inquiries
- Knows PPBH programs and is able to answer general program information
- Determines initial clinical and financial eligibility
- Conducts psychosocial evaluation on each assigned admission, gathers data
- Coordinates with the treatment team, family and patients
