About The Position

Terros Health is seeking an Assessment Clinician for its Javelina location in Mesa, AZ. Terros Health is a healthcare company focused on the whole person, providing primary care and specializing in mental health and substance use treatment for over 50 years. The Assessment Clinician works closely with the Maricopa County Adult Probation Department, Bridging the Gap (BTG) Re-entry and Crisis Aftercare Programs, and other collateral community contacts to engage justice and crisis-referred clients. This role involves conducting comprehensive biopsychosocial assessments to determine diagnosis and treatment needs, developing Service Plans, and providing evidence-based therapeutic interventions. The position reports to the Supervisor of BTG Re-entry or Director of Justice Services.

Requirements

Must possess a Master’s degree, i.e., MA, MC, MSW, in a behavioral health field.
Must have an Arizona behavioral health license (other than LBSW) or be eligible for Arizona license from Arizona Board of Behavioral Health Examiners.
Must have the ability to perform first aid or cardiopulmonary resuscitation as necessary and appropriate.
Knowledge in use of Electronic Health Record (NextGen a plus).
Proficiency using the MS Office suite, particularly Word and Outlook.
Must maintain a valid Arizona driver’s license, be 22 years of age with minimum 3 years driving experience, and meet requirements of Terros Health’s driving policy.
Must pass background check in accordance with Terros Health and DCS requirements (no probation or parole within the last 10 years).
Must have Arizona Fingerprint Clearance card or apply for one within 7 working days of assuming role.
Must pass DCS Central Registry check without any disqualifying events.
Must pass a TB Test.
Experience working with diverse populations.

Responsibilities

Reviews referral and supplemental documents prior to assessment to gain insight about the patient.
Conducts a comprehensive biopsychosocial and ASAM Criteria® assessment to identify treatment needs and level of care.
Requests additional information from the referral source if treatment is not recommended.
Coordinates with the referral source after each assessment, communicating recommendations and concerns.
Makes referrals to appropriate facilities for stabilization services if needed.
Prepares a written assessment summary detailing findings and treatment recommendations.
Completes coordination of care activities with other entities.
Connects clients to appropriate Terros Health team members based on referral source and identified needs.
Conducts therapeutic services using evidence-based interventions, which may include individual, couple, family, or group service methods.
