Assertive Community Treatment Licensed Practical Nurse (LPN)
About The Position
Carolina Outreach is seeking a Licensed Practical Nurse (LPN) for their Assertive Community Treatment (ACT) team. This role involves monitoring and overseeing the medical/clinical aspects of services, providing medication monitoring and documentation, and delivering psychopharmacological and medical services within the scope of practice. The LPN will collaborate with ACT team members, conduct crisis assessments, provide individual counseling, substance abuse treatment, and psycho-educational support. They will also assist in creating transition plans, act as a consultant to healthcare providers, ensure proper documentation, and be available for crisis response on a 24/7/365 basis. Requirements
- Active LPN license in North Carolina.
- At least 1 year of experience working with the identified population.
Responsibilities
- Monitor and oversee all medical/clinical aspects of services delivered.
- Provide medication monitoring and documentation.
- Deliver psychopharmacologic and medical services within your scope of practice.
- Collaborate with ACT team members to monitor clinical status and response to treatment.
- Conduct crisis assessments, symptom management, and intervention planning.
- Provide individual counseling, substance abuse treatment, and psycho-educational support.
- Assist in creating transition plans when clients move in and out of services.
- Act as a consultant to primary healthcare providers and community medical staff.
- Ensure proper documentation, including treatment plans and progress notes.
- Be available for crisis response and support to the ACT team on a 24/7/365 basis.
