Assertive Community Treatment Licensed Practical Nurse (LPN)
About The Position
Carolina Outreach is seeking a Licensed Practical Nurse (LPN) for its Assertive Community Treatment (ACT) team in Gastonia, NC. Since 2003, Carolina Outreach has been dedicated to improving the behavioral health of children, families, and adults across North Carolina. This role involves monitoring and overseeing the medical/clinical aspects of services, providing medication management, and delivering psychopharmacological and medical services within the scope of practice. The LPN will collaborate with ACT team members, conduct crisis assessments, provide individual counseling, and assist with client transitions. This position also involves acting as a consultant to primary healthcare providers and ensuring proper documentation. The role requires availability for crisis response on a 24/7/365 basis.
Requirements
- Active LPN license in North Carolina.
- At least 1 year of experience working with the identified population.
Responsibilities
- Monitor and oversee all medical/clinical aspects of services delivered.
- Provide medication monitoring and documentation.
- Deliver psychopharmacologic and medical services within your scope of practice.
- Collaborate with ACT team members to monitor clinical status and response to treatment.
- Conduct crisis assessments, symptom management, and intervention planning.
- Provide individual counseling, substance abuse treatment, and psycho-educational support.
- Assist in creating transition plans when clients move in and out of services.
- Act as a consultant to primary healthcare providers and community medical staff.
- Ensure proper documentation, including treatment plans and progress notes.
- Be available for crisis response and support to the ACT team on a 24/7/365 basis.
