Requirements

Current Kansas Licensed Practical Nurse (LPN) license in good standing.
Valid driver's license with a clean driving record.
Current automobile insurance.
Reliable transportation and willingness to travel throughout the community.
Ability to pass a vehicle inspection or provide proof of inspection completed within the past year.
Excellent communication and relationship-building skills.
Strong assessment and critical thinking abilities.
Compassion and respect for diverse cultures and backgrounds.
Ability to remain calm and effective during crisis situations.
Strong organizational and documentation skills.
Ability to work independently while collaborating with a multidisciplinary team.

Nice To Haves

Experience working in behavioral health, community mental health, or psychiatric nursing.
Experience supporting individuals with severe and persistent mental illness (SPMI).
Experience working with individuals who have co-occurring mental health and substance use disorders.
Knowledge of community-based services, care coordination, and evidence-based behavioral health practices.

Responsibilities

Provide nursing care in clients' homes and other community settings, including: Nursing assessments, Medication administration, Medication education, Health monitoring
Coordinate care with psychiatrists, primary care providers, hospitals, pharmacies, and community partners to ensure continuity of care.
Serve as an advocate for clients by helping them access medical, behavioral health, and community resources.
Educate clients and their families about medications, physical health, mental health conditions, and wellness strategies.
Assist with crisis intervention and emergency response when needed by coordinating with crisis services, emergency responders, supervisors, and medical providers.
Participate as an active member of the ACT team, attending team meetings and collaborating to develop individualized treatment plans.
Help clients develop skills to improve independence and manage emotional distress.
Build trusting, recovery-focused relationships with clients and their families.
Complete documentation accurately and in a timely manner.
Transport clients and provide services throughout the community as needed.
