Requirements

  • Current Kansas Licensed Practical Nurse (LPN) license in good standing.
  • Valid driver's license with a clean driving record.
  • Current automobile insurance.
  • Reliable transportation and willingness to travel throughout the community.
  • Ability to pass a vehicle inspection or provide proof of inspection completed within the past year.
  • Excellent communication and relationship-building skills.
  • Strong assessment and critical thinking abilities.
  • Compassion and respect for diverse cultures and backgrounds.
  • Ability to remain calm and effective during crisis situations.
  • Strong organizational and documentation skills.
  • Ability to work independently while collaborating with a multidisciplinary team.

Nice To Haves

  • Experience working in behavioral health, community mental health, or psychiatric nursing.
  • Experience supporting individuals with severe and persistent mental illness (SPMI).
  • Experience working with individuals who have co-occurring mental health and substance use disorders.
  • Knowledge of community-based services, care coordination, and evidence-based behavioral health practices.

Responsibilities

  • Provide nursing care in clients' homes and other community settings, including: Nursing assessments, Medication administration, Medication education, Health monitoring
  • Coordinate care with psychiatrists, primary care providers, hospitals, pharmacies, and community partners to ensure continuity of care.
  • Serve as an advocate for clients by helping them access medical, behavioral health, and community resources.
  • Educate clients and their families about medications, physical health, mental health conditions, and wellness strategies.
  • Assist with crisis intervention and emergency response when needed by coordinating with crisis services, emergency responders, supervisors, and medical providers.
  • Participate as an active member of the ACT team, attending team meetings and collaborating to develop individualized treatment plans.
  • Help clients develop skills to improve independence and manage emotional distress.
  • Build trusting, recovery-focused relationships with clients and their families.
  • Complete documentation accurately and in a timely manner.
  • Transport clients and provide services throughout the community as needed.
