About The Position

Envision Health Inc. is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our multidisciplinary Assertive Community Treatment (ACT) Team. The ACT LPN provides community-based nursing support and medication management services to adults with serious mental illness (SMI), co-occurring disorders, and complex behavioral health needs. This role supports the ACT Registered Nurse and psychiatric providers in delivering integrated, person-centered care in accordance with Virginia DBHDS, DMAS, and TMACT standards.

Requirements

  • Active Licensed Practical Nurse (LPN) license in the Commonwealth of Virginia required
  • Valid Virginia driver’s license with reliable transportation required
  • Ability to provide services throughout the community setting daily

Nice To Haves

  • Minimum of one (1) year experience in behavioral health, psychiatric nursing, community health, or substance use services preferred
  • Knowledge of psychotropic medications and chronic behavioral health conditions preferred
  • Strong communication, organization, and interpersonal skills
  • Ability to work effectively within a multidisciplinary treatment team

  • TMACT-informed service delivery
  • Substance Use Disorder (SUD) treatment experience

Responsibilities

  • Provide community-based nursing services and support to individuals enrolled in ACT services
  • Assist with medication management, medication monitoring, and medication education
  • Administer medications and injections as ordered under the supervision of the RN and/or prescribing provider
  • Monitor individuals for medication effectiveness, side effects, and overall wellness
  • Assist clients in understanding diagnoses, treatment plans, and medication regimens
  • Coordinate with pharmacies, medical providers, hospitals, and treatment teams regarding medication and healthcare needs
  • Participate in daily ACT team meetings and treatment planning discussions
  • Conduct wellness checks and support individuals with medical appointment follow-up
  • Assist with crisis response and stabilization interventions as directed by the ACT team
  • Support transitions from hospitals, residential placements, shelters, or correctional settings
  • Maintain timely and accurate documentation within the electronic health record (EHR)
  • Ensure confidentiality and compliance with HIPAA, DBHDS, DMAS, and agency policies
  • Participate in after-hours on-call rotation as required
