Provides support to both ASPIRE supervisor and supports coordinators including; professional development and training/observation of employee's job duties and functions, management of therapy referrals, management of psychiatric authorizations, assist with case discharge documentation, review and approval of guardianship paperwork, scheduling initial treatment planning appointments, report data to team on past due reviews and documentation, provides support to the team members in supervisor’s absence. Provides supports coordination services caseload of 35 to children and adults with intellectual and developmental disabilities to arrange, manage and provide supervision of support and services using person-centered planning principles. Enhance continuity of services with multiple providers (DHS, schools, vocational services) Supports coping skills Ensures consumers are receiving eligible SSI and Medicaid benefits Completes assessments, diagnosis and evaluations Participates in developing Person-Centered Planning meetings and I.P.O.S. treatment plans Acts as consumer advocate Complete assessments to identify each person’s needs and desires. This will include the use of formal and informal tools to gather information needed to the person to identify their needs and wishes. Has awareness of CNS Healthcare and all other applicable policies and procedures. Attends training and continues to remain knowledgeable about current best practices in services to support people with intellectual and developmental disabilities and co-occurring diagnoses. Trains families and consumers on their rights and due process. Maintains and updates clinical knowledge by readings, in-service training in designated areas and other activities in consultation with clinical supervisor. Complete at least 24 hours of in-service training on annual basis. Required to meet productivity standards for program and maintain required utilization review score. Provides comprehensive and coordinated care within the framework of the agencies integrated health care practice model Completion of all other duties that are needed to support the consumers and their family.
