About The Position

This role provides support to both ASPIRE supervisors and supports coordinators, including professional development, training, and observation of employee job duties and functions. The position involves managing therapy referrals, psychiatric authorizations, and assisting with case discharge documentation. It also includes reviewing and approving guardianship paperwork, scheduling initial treatment planning appointments, reporting past due reviews and documentation to the team, and providing support to team members in the supervisor’s absence. The role also entails providing supports coordination services for a caseload of 35 children and adults with intellectual and developmental disabilities, arranging, managing, and supervising support and services using person-centered planning principles. Responsibilities include enhancing continuity of services with multiple providers (DHS, schools, vocational services), supporting coping skills, ensuring consumers receive eligible SSI and Medicaid benefits, completing assessments, diagnoses, and evaluations, and participating in developing Person-Centered Planning meetings and I.P.O.S. treatment plans. The position acts as a consumer advocate, completes assessments to identify needs and desires using formal and informal tools, and maintains awareness of CNS Healthcare and applicable policies. The role requires attending training to stay knowledgeable about best practices in services for individuals with intellectual and developmental disabilities and co-occurring diagnoses, and training families and consumers on their rights and due process. The position also involves maintaining and updating clinical knowledge through readings and in-service training, completing at least 24 hours of in-service training annually, meeting productivity standards, and providing comprehensive and coordinated care within the agency's integrated healthcare practice model. The role also includes completing other duties as needed to support consumers and their families.

Requirements

  • Qualified Intellectual Disability Professional (QIDP)
  • Bachelor’s degree in Social Work, OR a Master’s degree in Social Work, Counseling, Psychology, or Marriage and Family Therapy, OR State licensure: Limited-Licensed Bachelor’s Social Worker (LLBSW), Licensed Bachelor’s Social Worker (LBSW), Limited-Licensed Master’s Social Worker (LLMSW), Licensed Master’s Social Worker (LMSW), Limited-Licensed Professional Counselor (LLPC), Licensed Professional Counselor (LPC), Limited-Licensed Marriage and Family Therapist (LLMFT), Licensed Marriage and Family Therapist (LMFT), Temporary Limited-License Psychologist (TLLP), Limited-Licensed Psychologist (LLP), OR Licensed Psychologist (LP)
  • Individual with specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons with intellectual or development disabilities as part of that experience), OR At least 1 year of experience in training or working with a person who has an intellectual disability
  • Either currently possess or obtain the following professional designations within 1 year of employment: Child Mental Health Professional (CMHP) and Qualified Mental Health Professional (QMHP)
  • Valid and unrestricted Driver’s License
  • Transportation or Automobile accessibility with current automobile/vehicle insurance meeting agency policy requirements
  • Proficient with computers, Electronic Health Record (EHR) systems, and Microsoft office products including Word, Outlook, PowerPoint, Excel, SharePoint, and Teams.

Nice To Haves

  • Prior experience working with all age groups including children; solution focused brief therapy; substance use and co-occurring disorders treatment; stages of change and/or motivational interviewing.
  • Prior experience in a community mental health setting.
  • Working knowledge and understanding of intellectual and developmental disabilities, co-occurring mental illness or other behavioral health disorders, medications used in treating behavioral health conditions, and experience with other interventions used to treat behavioral health conditions.
  • Ability to share and teach about the symptoms of substance use disorders that are similar to those of other medical and/ or psychological disorders and how these disorders interact

Responsibilities

  • Maintains and updates clinical knowledge by readings, in-service training in designated areas and other activities in consultation with clinical supervisor. Complete at least 24 hours of in-service training on annual basis.
  • Required to meet productivity standards for program and maintain required utilization review score.
  • Provides comprehensive and coordinated care within the framework of the agencies integrated health care practice model
  • Completion of all other duties that are needed to support the consumers and their family.
