About The Position

This position practices in the Hospice program and provides patient care in collaboration with a physician. Care includes diagnosis, appropriate treatment of disease processes, symptom management, and follow-up care for terminally ill patients. The role is 1.0 FTE, with four 10-hour shifts on a variable schedule, mostly weekdays, and variable on-call coverage of 1–2 overnight shifts per week.

Requirements

• Master’s degree as an ARNP.
• Certification in Hospice, Family Medicine, or Palliative Care.
• Graduate of an accredited medical school.
• Licensed in the state of Washington by the time of employment.
• DEA, NPI, and prescriptive authority.
• Current BLS for Healthcare Providers certification from the American Heart Association.

Responsibilities

• Perform face-to-face assessment screenings of Hospice patients based on Medicare Hospice eligibility criteria.
• Prescribe medication doses, routes, and frequencies based on individual patient assessments.
• Actively manage Hospice patients in association with the Hospice Medical Director at every level of care: Routine, General Inpatient, Respite, and Continuous Care.
• Develop treatment plans based on scientific rationale, standards of care, and professional practice guidelines.
