About The Position

The position emphasizes health promotion, disease prevention, and management of acute and chronic illnesses across various patient populations. The role also includes evaluating care processes and outcomes through quality improvement efforts, engaging in professional development, collaborating with medical staff and healthcare providers, performing emergency treatments, maintaining documentation, and communicating effectively within the healthcare team.

Responsibilities

  • Utilizes advanced nursing processes for patient care, including assessment, planning, intervention, implementation, and evaluation.
  • Performs initial and ongoing needs assessments of patients and families to identify health risks and needs.
  • Provides primary, secondary, and tertiary healthcare, emphasizing health promotion, disease prevention, early detection, management of health problems, and response to illness.
  • Formulates differential diagnoses and develops comprehensive, multidisciplinary plans of care.
  • Prescribes interventions and performs therapeutic measures to implement individualized plans of care.
  • Modifies the plan of care in collaboration with healthcare team members.
  • Participates in health education for patients, families, communities, healthcare providers, and students.
  • Evaluates patient progress toward desired outcomes.
  • Demonstrates personal accountability in delivering competent, professional, and compassionate patient care.
  • Identifies and recognizes abnormal symptoms/changes in patient condition, establishes priorities, and takes appropriate action.
  • Evaluates the effectiveness of the plan of care for aggregate populations.
  • Analyzes the appropriateness of resource utilization.
  • Evaluates cost and quality outcomes of care provided.
  • Utilizes outcome data to enhance the delivery of quality, cost-effective healthcare.
  • Coordinates and/or provides formal educational programs.
