About The Position

InterCommunity is a Federally Qualified Health Center Look-Alike (FQHC LA) committed to providing accessible, compassionate care to everyone — regardless of life situation or ability to pay. We offer same-day primary care and a wide range of behavioral health services across our community health centers. Our Addiction Services Division provides a full continuum of care, including primary care integration, residential detox and treatment, outpatient mental health and substance use services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services, and social rehabilitation. This role is accountable for providing advanced practice nursing and consultation services in accordance with written standardized procedures, managing the primary care needs of all SBHC patients.

Requirements

  • Master’s degree in Nursing

Responsibilities

  • Provides back to school physicals, sports physicals, immunizations, urgent care, sick visits and other requested services to all children who are registered with the SBHC.
  • Provides appropriate nursing and/or health care services to patients including obtaining complete histories; performing pertinent physical exams with assessment of normal and abnormal findings on new patients; clinical assessments, treatments, diagnosing medical conditions, assessing emergencies, ordering diagnostic studies, prescribing medications.
  • Obtains interval histories and performs pertinent examinations on return patients.
  • Administering injection.
  • Obtaining PAP smears, genital, wound and other cultures as indicated.
  • Performing phlebotomy.
  • Performs specialized duties in providing medical care to a variety of patients including individuals with acute and chronic conditions.
  • Consults with clinical staff regarding assessment methods and treatment plans.
  • Assists in determining need for contractual health services.
  • Provides direction to staff on appropriate health record documentation.
  • Prepares reports on health services, problems and investigations.
  • Participates in quality assurance and other clinical committees as needed.
  • May assist families and other groups in understanding medical needs of patients serviced by agency; may participate as a member of multi-disciplinary evaluation team.
  • Recognizes situations which require the immediate attention of a physician and initiates life-saving procedures when necessary.
  • May provide training to nursing and paraprofessional staff; may direct agency clinic services.
  • All agency staff are required to attend all mandatory department/agency meetings and trainings.
  • Escort patients to exam room, perform vital signs, blood sugar testing, EKG’s, urinalysis, phlebotomy, spirometry etc as instructed by provider.
  • Ensures lab results are reviewed and signed by provider and disseminated as needed.
  • Will participate as a member of an interdisciplinary treatment team in the assessment, development, implementation and evaluation of client’s treatment or health/mental health care plans.
  • Maintains medical office instruments, equipment, and supplies and ensures cleanliness of equipment and environment.
