About The Position
Cypress at Home is seeking an experienced Nurse Practitioner or Physician Assistant to provide quality care to older adults. The role provides comprehensive health care in a health care clinic, skilled nursing facility, assisted living facility, and private homes. The clinician works with geriatric patients to develop and implement comprehensive plans of care, collaborating with other health team members to achieve desired patient outcomes. The role also serves as a leader and role model for the professional development of peers and others. Cypress at Home supports seniors in remaining independent in their homes while helping them achieve the highest quality of life possible. Its care management program has been in existence for over 25 years and was originally part of the Lee Memorial Health System.
Requirements
- Graduate from a Nurse Practitioner or Physician Assistant program.
- Valid Nurse Practitioner or Physician Assistant license.
- Valid driver's license.
- Previous gerontology experience.
- Knowledge of state and federal rules and regulations regarding patient care in a skilled nursing facility.
- Effective verbal and written communication skills.
- Interpersonal skills necessary to work productively with patients, families, nursing staff, and other departments.
Responsibilities
- Provide comprehensive health care to residents in the health care clinic, skilled nursing facility, assisted living facility, and private homes.
- Develop and implement comprehensive plans of care for geriatric patients, including assessment, diagnosis, treatment, management, education, health promotion, and care coordination.
- Collaborate and coordinate with other health team members to achieve desired patient outcomes.
- Serve as a leader and role model for the professional development of peers and others.
