About The Position

The DH-Kendal Clinic, located within the Continuing Care Retirement Community of Kendal at Hanover, is seeking an enthusiastic and experienced APRN or PA to join their team. The clinic provides a broad spectrum of geriatric primary care and chronic disease management. Routine care is delivered through clinic visits, independent living home visits, and assisted living Health Center visits. Acute issues are also addressed urgently within these settings. The role emphasizes wellness and palliative care for approximately 400 residents aged 65 and older. The clinic provider team consists of one physician and two nurse practitioners. The ideal candidate will collaborate closely within an interdisciplinary team including nursing, social work, rehab, nutrition, and spiritual care. Excellent and professional communication skills with both patients and DH specialists are crucial, as is autonomous clinical skill for periods of working alone. On-call responsibilities are part of this role.

Requirements

Graduate of an accredited APRN or PA academic program
National specialty certification as FNP, WHNP, PNP, ANP or certification through the National Commission on Certification of Physician Assistants (NCCPA)
Eligible for RN licensure in the State of NH (APRN only)
Eligible for APRN or PA licensure in the State of NH
Eligible for Federal DEA Registration
BLS certification

Responsibilities
