About The Position

Reporting to the Lead Community Health Worker, the Community Health Worker (CHW) assist patients and the surrounding community with maintaining and improving health. The CHW serves as a link between Total Health Care (THC) patients, the health center and other community resources. The CHW assist patients and the surrounding community with navigating healthcare systems, accessing needed services and recognizing potential health challenges that may lead to poor health and social outcomes. The CHW also plays an essential role in assisting THC in achieving and/or exceeding value based reimbursement payments. This position also supports THC’s community outreach activities, events, and initiatives engaging with both partners and the community at large. Contacts and interactions vary and may involve multiple stakeholders such as direct interaction with THC’s executive leadership, community organizers, the general public, THC’s patients, physicians, colleagues, assigned staff, vendors, contractors and consultants for the purpose of providing and exchanging information.

Requirements

  • Must be able to communicate effectively with diverse populations.
  • Must possess strong interpersonal skills.
  • Must be able to work independently and as part of a team.
  • Must be able to maintain confidentiality.
  • Must be proficient in Microsoft Office Suite.

Nice To Haves

  • Bilingual in Spanish.
  • Experience working in a community health setting.
  • Knowledge of local community resources.
