Requirements

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program.
  • Active, unrestricted Arizona license.
  • At least 2 years of experience providing clinical services to pediatric (including newborns) and adult patients with chronic medical and behavioral health conditions (preferred).
  • Proven skills, knowledge base, and judgment necessary for independent clinical decision-making and problem-solving.
  • Hands-on clinical skills, including venipuncture, patient assessment, and chronic care support, or willingness to learn.
  • Ability to work at least 1-2 days per week (8-10 hours per day).
  • Current BLS/CPR certification.
  • Passion for serving underserved and vulnerable populations with empathy and respect.

Responsibilities

  • Provide in-person primary care visits as part of a comprehensive longitudinal care model, working in close collaboration with a virtual PCP.
  • Conduct adult and pediatric wellness visits addressing quality care gaps and establishing baseline physical exams.
  • Provide acute and chronic care, including transition of care visits after hospitalizations.
  • Reconcile and prescribe medications, order and administer labs and diagnostics, and refer to specialist care.
  • Partner with an interdisciplinary team, including MDs, LCSWs, RNs, psychiatrists, and medical assistants.
  • Document care and maintain clear, comprehensive care plans.
  • Help families identify and access health and social care resources.
  • Participate in community outreach efforts.
