Nest Health is a healthcare startup founded in 2022 with the mission to make comprehensive medical, behavioral, and social care radically accessible to America’s highest risk families. Requirements

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program with an active, unrestricted Arizona license
  • At least 2 years of experience providing clinical services to pediatric (including newborns) and adult patients with chronic medical and behavioral health conditions (preferred)
  • Proven skills, knowledge base, and judgment necessary for independent clinical decision-making and problem-solving
  • Hands-on clinical skills, including venipuncture, patient assessment, and chronic care support, or willingness to learn
  • Ability to work at least 1-2 days per week (8-10 hours per day), with the opportunity to expand hours as the program grows
  • Current BLS/CPR certification
  • Passion for serving underserved and vulnerable populations with empathy and respect

  • Leadership experience in an interdisciplinary care team preferred
  • Startup experience preferred

Responsibilities

  • Provide in-person primary care visits as part of a comprehensive longitudinal care model, working in close collaboration with a virtual PCP who serves as the patient’s primary care provider; virtual care modalities may be used as appropriate
  • Conduct adult and pediatric wellness visits that address quality care gaps and establish a comprehensive baseline physical exam for all patients
  • Provide acute and chronic care requiring physical examination or diagnostic testing, including transition of care visits after high-risk hospitalizations, as needed
  • Reconcile and prescribe medications, order and administer labs and diagnostics, and refer to specialist care, as needed
  • Partner with an interdisciplinary team, including MDs, LCSWs, RNs, psychiatrists and medical assistants upskilled to support social services navigation
  • Document care and maintain clear, comprehensive care plans addressing patient and family needs
  • Help families identify and access health and social care resources, including food, housing, transportation, and financial assistance
  • Participate in community outreach efforts, as needed
