About The Position

The Nurse Practitioner (NP) or Physician Assistant (PA) is responsible for clinical activities in the rural and/or urban healthcare facilities under the direction and supervision of a physician (off-site). The NP or PA utilizes independent judgment and directs the care and treatment of patients receiving integrated primary and behavioral health care. This position serves adult patients with acute and chronic medical issues.

Nice To Haves

  • Prior primary care or internal and/or geriatric medicine strongly preferred.
  • Prior management experience preferred.

Responsibilities

  • Assess, plan and provide a comprehensive plan of care that proactively manages healthcare issues and addresses barriers along the care continuum for patients in the nursing home setting.
  • Responsible for preparing monthly APP chart reviews and educating staff on best practices.
  • Work with the Director of Quality and Education on ways to support their market with continued education for best results.
  • Work with Director of Education and Quality on market specific quality initiatives and communication.
  • Provide medical care within the scope of the practitioner’s training; consult with supervising physician when necessary and appropriate.
  • Ensure compliance with local, state, and federal regulatory agencies related to clinical services provided by NPs or PAs.
  • Assess and determine appropriate medical and psychological intervention; prescribe medications and services as indicated.
  • Order appropriate laboratory studies, diagnostic procedures, and consultations.
  • Monitor compliance with medications; evaluate patient response to plan of treatment; modify plan of treatment when necessary.
  • Work with RN Case Managers as part of an integrated care team.
  • Communicate effectively and interact constructively with a diverse group of patients.
  • Maintain thorough and timely documentation of services rendered in the patient’s chart.
  • Document and provide correct billing information on all patient visits, consultations, and procedures to office staff.
  • Assist with on-call service with physician staff to provide 24/7 access to care for patients.
  • Participate in the review of quality related data linked to clinical practice and outcomes.
  • Conduct informal facility staff education and teaching to improve the effectiveness and efficiency of care delivery.
  • Act as subject matter expert when appropriate to support team success.
  • Maintain small patient caseload as needed.
  • Serve as mentor and/or trainer for new staff.
  • Perform administrative and other duties as required.
  • Other supervisory or leadership tasks as assigned by the clinical program manager or director.
