About The Position

This role involves providing direct patient care, including rooming patients, taking vital signs, and assisting with patient mobility. The nurse will perform patient assessments, ensure patient needs are met, and act promptly in emergencies. A key responsibility is identifying and coordinating Medicare Annual Wellness Visits (AWVs), working collaboratively with clinical teams to ensure efficient patient care. The role also includes patient advocacy, developing knowledge of Medicare preventive services and clinical guidelines, and addressing quality care gaps with patients. Accurate documentation in the EMR is essential for quality gap closure. The nurse will chaperone providers, administer medications and injections, and perform various clinical procedures such as EKGs, nebulizer treatments, and diagnostic tests. Pain management education, scheduling appointments and imaging, coordinating referrals, and obtaining insurance pre-authorizations are also part of the duties. Communication with providers, prescription refills, reviewing physician orders, and maintaining organized patient rooms are expected. The role also requires identifying and reporting compliance concerns and performing other assigned duties.

Requirements

  • Completes annual educational requirements within the assigned time frame.
  • Maintains regulatory requirements, including all state, federal and Joint Commission regulations related to their department and, as appropriate, to the hospital.
  • FL LPN or RN License required
  • BLS

Responsibilities

  • Room patients, obtain vital signs, and assist patient in and out of the clinic as needed
  • Perform patient assessments and reassessments with in scope of practice under supervision or RN and/or provider as needed
  • Ensure patient needs are consistently met, consulting with health team members as appropriate
  • Act promptly and effectively in emergency situations
  • Reviews clinic schedules and identifies patients appropriate and due for Medicare AWV
  • Works collaboratively with clinical teams to accomplish AWV in an efficient manner for patients and the team
  • Acts as a patient advocate
  • Develops knowledge of Medicare preventive services and clinical guideline recommendations for those services
  • Address open quality care gaps with patients and works collaboratively with patient towards closure of those gaps
  • Appropriately documents in the EMR to satisfy closed quality gaps
  • Chaperone providers during examinations and assist with procedures as needed
  • Administer medications and injections with accurate documentation as needed
  • Perform clinical procedures including EKGs, nebulizer treatments, fingerstick glucose testing urinalysis, and fecal occult blood testing with accurate documentation as needed
  • Apply appropriate pain management techniques and educate patients and families with accurate documentation
  • Provide age- appropriate care based on established standards
  • Schedule patient appointments and imaging studies in accordance with clinic policy
  • Coordinate referrals to specialists and other healthcare providers
  • Obtain insurance pre-authorizations as required
  • Communicate patient information via phone to providers in a timely and accurate manner
  • Call in prescription refills as authorized by providers
  • Review physician orders and carry out new orders accurately
  • Ensure all assigned tasks and responsibilities are completed by the end of each workday
  • Maintain clean, stocked, and organized patient rooms as needed
  • Identify and report compliance concerns through appropriate channels
  • Orders patient appointments and studies according to policy
  • Perform other duties as assigned by the office manager or provider
