About The Position

The Ambulatory Wilmot Oncology Nurse Navigator at Strong: Radiation Oncology Nurse Navigator role is a full-time position primarily focused on days with 8-hour shifts. This role is available within the progressive outpatient clinics and seeks a team-focused professional passionate about caring for radiation oncology patients. The position aims to optimize care coordination, enhance patient experience, and improve clinical outcomes by providing nurse-led navigation throughout the patient journey. The navigator will collaborate with appropriate teams to ensure scheduled tests, diagnostic procedures, follow-up care, and appointments are coordinated within the radiation oncology clinical pathway. They will also provide education to patients and families regarding complex appointments and required testing. The navigator is responsible for identifying and addressing clinical and health system barriers to care and the provision of services to at-risk populations, as identified by nurse navigation tracking tools. Additionally, the navigator will assist in developing clinical goals and objectives for the Radiation Oncology Navigator role in conjunction with the radiation oncology service. Excellent communication skills and the ability to work independently with multidisciplinary teams are required for success in this position.

Responsibilities

  • Provides cancer disease-specific individualized assistance, support, and comprehensive coordination of care for patients and their families/caregivers to eliminate barriers to timely care, facilitate flow through the system, facilitate interactions with the care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as the point of contact for patients and families throughout the cancer continuum.
  • Works in collaboration with the care team and other multidisciplinary teams to establish, execute, and evaluate a care plan across the system for URMC patients.
  • Optimizes care coordination, patient experience and improves the clinical outcome for patients by providing nurse-led navigation throughout the patient journey.
  • Collaborates with the appropriate teams to ensure that scheduled tests, diagnostic procedures, follow-up care, and appointments are coordinated within the radiation oncology clinical pathway.
  • Provides education to patients and families regarding complex appointment and required testing.
  • Identifies and addresses clinical and health system barriers to care and provision of services to at-risk populations as identified by the nurse navigation tracking tools.
  • Assists in the development of clinical goals and objectives for the Radiation Oncology Navigator role in conjunction with the radiation oncology service.
