About The Position

This position plays a vital role in supporting patients prescribed oral oncology treatments within the Oncology Clinic. The Ambulatory Pharmacist ensures continuity of care by monitoring lab results, managing adverse effects, performing medication reconciliation, and providing counseling to patients. Collaboration with the specialty pharmacy team is essential to address medication access and availability. Additionally, this role serves as a trusted clinical resource to providers and nursing staff.

Requirements

PharmD from a College of Pharmacy accredited by American Association of Colleges of Pharmacy, or equivalent clinical experience required.
Current Montana State Board of Pharmacy, Registered Pharmacist License required.
Accurate data entry skills, analytical and advanced math skills required.
RQI required and as applicable to assigned area(s).
First Responder required for assignment: Clinic, Retail, Behavioral Health, IS and Administration Pharmacists.
ACLS required for all other assigned area(s).

Nice To Haves

Experienced in ambulatory care pharmacy settings
Knowledgeable in oncology, especially managing specialty medications
Oncology Pharmacy Specialty Certification (BCOP) preferred
Completion of PGY2 in Ambulatory Care, Oncology or related area preferred
Ideally completed a PGY2 residency in Ambulatory Care, Oncology, or related area
BCOP certified or actively pursuing Oncology Pharmacy Specialty Certification

Responsibilities

Evaluates and verifies orders for therapeutic appropriateness, proper dosing, optimal dosage form, drug interactions, allergic reactions, route and rate of administration, and therapeutic redundancy.
Provides patient-specific pharmacotherapy recommendations and drug information to the appropriate stakeholders.
Performs daily review of pharmacy reports including pharmacokinetic monitoring.
Documents clinical interventions appropriately in electronic medical record.
Implements all drug use policy and formulary decisions of the Pharmacy and Therapeutics committee.
Reviews and updates Collaborative Practice Agreements per department protocol.
Supports and precepts pharmacy students on practice rotations.
Assesses and evaluates the medical and drug literature. Compares and contrasts therapeutic regimens and uses for new or existing drug treatments.
Provides direct and indirect patient care for patients through Collaborative Practice Agreements.
Develops and participates in operational and clinical projects as determined by departmental needs and ongoing projects.
Performs direct patient care rounding, order verification, discharge counseling, and education of other healthcare professionals.
Supervises the pharmacy technicians.
Serves as a preceptor for pharmacy students on rotations.
Maintains pharmacy knowledge through continuing education and applies new principles to current practice as needed.
Exhibits knowledge of policies and procedures as they relate to job duties and HIPPA.
Coordinates medication ordering, preparation, dispensing, and administration systems for the oncology patient populations.
